4/15/20 Update:

This PREPRINT (not yet peer-reviewed) study which was made available online on 4/14 (doi: https://doi.org/10.1101/2020.04.10.20060699) is perhaps the best quality evidence thus far.  Investigators in France took data from patients in 4 hospitals who underwent routine care.  Inclusion included admission to one of four hospitals, age 18-80, PCR-confirmed SARS-CoV-2 infection, and oxygen by mask or nasal prongs.  Important exclusions included organ failure necessitating ICU admission and ARDS (definied by need for NIPPV or mechanical ventilation).  Authors used multiple variables (age, gender, BMI >30, multiple comorbidities, etc) in order to work backwards and estimate each patient’s probability of receiving HCQ.  This approach basically allows the authors to create a synthetic sample where treatment assignment becomes independent of these baseline covariates (emulating randomization).  (Specifically, they used an inverse probability of treatment weighting (IPTW) which is one of the methods for propensity scoring).

The primary outcome was a composite: transfer to the ICU within 7 days of inclusion and/or death from any cause. Secondary outcomes were all-cause mortality at day 7 and the occurrence of ARDS.

181 patients were eligible for the analysis, 84 received HCQ within 48 hours of hospitalization and 97 did not.

Results demonstrated no significant differences between groups, as summarized in this table (that is, no significant differences in primary composite outcome and secondary outcomes).
